What Is a Deans Form and When Should I Get It?

A Dean's Form is a document provided by CAMHS that allows for the Ad Board to consider make-up exams or extensions beyond the final exams period due to a medical condition (be it acute and/or ongoing). You should not go to Urgent Care for a deans form but rather, reach out to your Resident Dean and make a Same-Day appointment.
