Pass/Fail Form Protocol

Follow These Steps For A Smooth Transition From Pass to Grade or Grade to Pass!

1. Go to your my.harvard.

2. Go to the "Document Center" and see a "Change of Grade Petition" Form (this will only show up for courses you are already enrolled in!)

3. Print the form and

1. request a signature from your instructor,

2. then your advisor, and

3. then submit it to me.

You can (preferably) send it to me electronically where i will add my digital signature and then upload to the Registrar. I will NOT sign the form without your instructor and advisor's signatures.

You should see the change reflected in your my.harvard within two weeks from submission.
